 Sometime in our life there are some cases which actually astonish us and we could not even believe it. But yes, it happens in our life. Same thing happened with me through a very interesting case. When a case surprised the physician it means it makes the physician more believable in his pathy. Just like that my believe in homoepathy has enhanced so much from 100 to 110%. 

It was just 10.30 A.M in the morning when I was busy in my newspaper as per my daily routine and then a old lady came for the follow up consultation. Then I wrapped up my paper & put it on the table & then I turned my chair towards the patient, saw another young lady was there in waiting area through my cabin glass. That I did not know she came with my follow up patient. Now I changed my focus from that lady to my follow up patient, she was improving so very quickly I completed the follow up consultation with the prescription. And this old lady case is also interesting one but that I would tell you in some other time. Now after finishing her prescription she told me, Doctor-you know she is my daughter-in-law and she has  a very "bad foot"( uska pair kharab ho raha hai) .

Then I started the proper consultation for that young lady who was in full Islamic hizab from head to foot only her face is open with hands & legs. From that open parts like face, hands & foot there is imparting of redness like skin is glowing very pinkish red just like full of blood. And I started proper case history by asking a very simple  question like how this occurred? Give me some associated history.Then she started detailing her history from beginning.Now a days a  homeopath is very fortunate enough if he gets such  kind of patient who explains minor things to the doctor very clearly. And in this case I were.

" Few years back I have some cracks on my both the soles & it was very hurting at that time. So, without consulting any doctor I just take OCT medicine & one ointment as suggested by chemist and applied for 1-2 weeks as suggested by chemist. After  a week it was like 90% better &almost gone & before completing the 2nd week its gone forever. This is all about the history doctor but I was worried when I saw it again on one of my foot and very quickly it appeared in my both the soles. And it getting worse for last 8 months and it became like that. You know doctor,  I d'not want to see my foot anymore because of that condition." 

This is the history that patient had shared and from here we can clearly understood one thing about the patient is that she is very conscious for her look her beauty. And this led me to think about the sulphur and I cross checked her details history to confirm the remedy. Then through all the parameters sulphur was selected &  Sulph 0/1 was prescribed for 21 days. 

After 21 days I was desperately waiting for her follow up date but days & months have gone & that lady did not come for the follow up consultation for about 6 months. Then slowly & gradually that patient wiped out from my mind & we never follow up that lady. But after 7-8 months later that patient again visited our clinic but this time not for herself, she came for her child who had some fungal infection over the scalp called tinia capitis. When she entered into the cabin, she first gave a smile & from that smile I remembered her but I did not ask about her foot this time because even she did not tell anything about the same. Then she explained her child case & we as per the totality prescribe one remedy for her child. At the close of the prescription she asked doctor gave him some good medicine like me. By saying this she stroked me to ask about her foot. And then I asked how is your foot now? she gave me a curing smile on her face & told that it had gone much earlier with you medicine & I did feel to come again for my foot. "Sciatica & Homoeopathy"

 Definition: Pain through any of the sciatic nerve is called Sciatica. It generally affects only one side of the legs. It starts through the course of sciatic nerve like from buttock to leg. Generally it affects one leg at a time. Sciatic nerve is the longest nerve in the body.

*Pain is generally due to compression of the sciatic nerve from herniated disc, bone spur or narrowing

of the spine. Patient may also feel numbness in the affected leg. 

*The word "Sciatica" is exactly used for pain originated due to injury to the sciatic nerve but now a days any pain started from lower back to the leg is called sciatica.

*Sciatica is a common complaints that people are suffering from. Almost 40% people suffer from it. And its a symptom not the condition. 

 *Most of the pregnant ladies may get the sciatica during their pregnancy because of hormone which causes loosening of the ligaments resulting unstable spine.And finally chances of slip disc increased.

*Anyone between the age group of 30-50 yrs may get the sciatica. 

Symptoms: 

1. Location-Pain starts from the lumbar region of spine & radiates to buttock & down towards the leg. This is the "Hallmark of Sciatica". Sometime pain also radiates to front of the leg.

1. Character- There will be an excruciating kind of pain from mild to severe . Sometime patient has burning sensation and sometime its like electric shock.Pain  may be sometime constant or comes & goes suddenly. 
2. Sensation- There is a numbness or tingling sensation in the affected leg.

Causation: There are two(2) major reasons for compression of sciatic nerve to cause pain. 

1) Herniated Disc of spine 2)Spinal Stenosis and 3) Very rarely a Tumor in spine.

Herniated Disc of spine- This is the most common cause of the sciatica. It creates a pressure on the nerve root. Because of this pressure on the root the nerve, central part of the vertebral disc which is gel like,bugling out through the vertebra & compresses the sciatic nerve. About 1-5% people have slip disc at certain level in their life.

Spinal Stenosis- Spinal stenosis is a degenerative disc disease which reduces the space between the vertebrae. Because of this low available space, there would be pressure on the spinal cord & nerves. This is very common in old age group people.

Spondylolisthesis-When one vertebra overlaps over the another, this condition is called spondylolisthesis. Due to this, space for the nerve entry is reduced & it can pinch out the sciatic nerve. 
Trauma/Injury- Any trauma or injury to sciatic nerve may cause the sciatica. 
Tumor- Any tumor in the spine especially in lumbar area may compress the sciatic nerve and resulting in sciatica.

Piriformis Syndrome- This is very rare & uncommon neuromuscular syndrome in which piriformis muscle becomes tight enough placed deep in buttock.This muscle is placed directly above the sciatic nerve and it causes pressure or irritation on the sciatic nerve if gets affected like spasm.  

Risk Factors: 

1. Age- whatever problems  you are suffering from, there is always an age related risk factor  & same in sciatica too. With the advancement of age, there is narrowing of the spine resulting in bone spur causing the compression to the sciatic nerve resulting in pain. 
2. Obesity-Obesity is one of leading rick factor behind most of the problem like Hypertension, Diabetes etc. Excess fat due to obesity creates a pressure upon the sciatic nerve resulting in sciatica. 
3. Occupation-Sometime your occupation may also play a risk factor for sciatica. Occupation like caring heavy loads, making your body twisting frequently may affect your sciatic nerve but it rarely happens. 
4. Sitting- Sitting for a long time in a position may also affect your sciatic nerve. This is found in slow active people more than in active people.
5. Diabetes-with diabetes chances of nerve damage increases & that increases the chances of sciatica.

Complication: In most of the cases people get recovered from sciatica either with home remedies or if not done, some simple treatment. Its very rarely complicated in following forms. 

1. Complete benumb condition of the affected leg resulting in Drop Foot.
2. Weakness of the affected limb 
3. Bowel or bladder function hampered 

Diagnosis: We may diagnose the sciatica with numbers of laboratory tests available as of now. But it all depends on the need that test the basis of the symptoms of the patient.

1. Straight Leg Raise Test-This is the clinical procedure where you have to lie on the floor straight overr you back on the floor & both legs should be straight. Then slightly raise your leg keep it stretched as maximum as you can, if you feel at certain point then  mark that point  because that may be the pin point for nerve involvement. 
2. X-ray- A simple pain x-ray of spine may figure out the bone spur or herniated disc.
3. MRI- With this procedure we can see the cross section images of the spine & a details study of bone and soft tissue.
4. Electromyography(EMG)-This is the confirmatory diagnosis to find out the narrowing of spine via impulses produces by the nerve & your muscles responses. 

Management & Treatment:

Managements

1. Exercise-Doing regular exercise is the most important preventive measure for sciatica. 
2. Posture-Sitting posture is also very important to maintain your muscles curve & alignments. 
3. Cold packs- Cold application over the affected area may relieve your pain. This should be done 3-4 times in a day.
4. Hot packs- Sometime hot application may also give some relieve. You can use it as per your comfort. 

Treatments

A)Conventional Mode of Treatment: 

Most of the consulting physicians practicing the old school medicine and give the muscle relaxants & medicines depending upon your level of your pain. 

Depending upon the cases anti-inflammatory drugs & corticosteroid injections are also given to relieve the pain &swelling of the affected nerve. 

If you are responding to the above the line of treatments, your physician may think about the surgery but its very rarely indicated. If your pain is at the severe level with extreme weakness of lower extremities & also you have somewhat lost the bladder & bowel control, then only your doctor may advise the surgery for you. But you doctor consider the surgery within a year of your sufferings. Now in this regards, two(2) kind of surgeries are considered:-

1)Microdisectomy-

   This kind of surgery is especially performed when the herniated disc is the cause of sciatic nerve. It is one the minimal invasive surgery via which a fragment of herniated disc is removed,which is responsible for creating a unnecessary pressure on the sciatic nerve. 

2)Laminectomy-

   This kind of surgery is performed to remove the lamina of the vertebra because in some cases it may cause the pressure on the sciatic nerve.

But people mostly prefer to take it from Old School of Medicine(Allopathy) as their first choice and when some percentage of them do not get any satisfied results , they would take a move to other options available in medical field.

"Boils & Abscess with Homoeopathy"

 Its very strange to know that people are still with doubt in their mind for Homoeopathy whether this pathy would help or not. However, its a fact to be known by everyone now that Homoeopathy is a proven science & it works. By seeing this scenario in this era, now I can clearly understand that our pathy has still a long way to go and that is a good thing because stagnation is a sign of decline. 

Why  I am saying this because a person came to my clinic with this kind of thought with his 4 year old daughter. Just after entering the Doctor cabin he asked me the very common question about her daughter's complaint, could it be cured???. And what a astonishing expression he had at that moment. 

This type of question comes to a person's mind from two(2) preferences, firstly, he had tried all the possible ways or pathies to get ride of the complaints from which her daughter suffering or he has no any believe in our pathy just to take a chance. Here, that person came from the first preference because he has tried all the possible ways for his daughter.

"Peripheral Artery Disease & Homoeopathy"

 Definition: Peripheral Artery Disease also named as Peripheral Arterial Disease is a clinical condition characterized by less blood flow to the lower limbs due to narrowed lumen of lower limbs arteries. Its one of the common circulatory problem of lower limbs. Its also called Peripheral Vascular Disease.

Features: 

1. It may also affect your arms but mostly the lower limbs are involved. 
2. Its a sign of fatty deposits in lumen of your arteries commonly known as atherosclerosis. 
3. It lowers down the blood flow to the kidneys,stomach & arms. 
4. Americans are mostly commonly affected by PAD than any other racial groups.
5. Males are frequently affected than females. 

Clinical Features: 

Mostly the people are not aware that they are suffering from PAD because its asymptomatic & they suddenly diagnosed with it as they face sudden cramp  & pain in lower limbs. 

1. Cramping may in hip & calf muscles after physical activities like walking, stretching arms or climbing something. You may also feel pain. 
2. Weakness and numbness of leg 
3. Coldness of the affected leg in compare to other leg .
4. Changes in the color of affected leg in compare to other. 
5. Hair loss in the affected leg or less growth of hairs in compare to other leg.
6. More glowing of skin in the affected leg.
7. Erectile Dysfunction in men .
8. Brittle toenails. 

Causes: 

Atherosclerosis: Atherosclerosis is the prime reason for the PAD. It cause shrinkage of lumen of arteries resulting in less flow blood. Usually it occurs in arteries of heart disease & all over the body but when it happens to limb, it causes PAD.

In atherosclerosis there is formation of plaque inside the lumen of arteries due to presence of fat & Cholesterol. Their formation process is very slow even we are unaware about. It makes our arteries harden & narrow & ultimately arteries lumen becomes smaller which would not allow to pass much blood flow.

     2.Risk Factors: There are certain factors which can enhances the condition.

-Family History of similar disease. 

-Diabetes

-High Blood Pressure

-High Cholesterol. 

-Smoking

-Obesity 

3.Complications: If PAD is caused by some deposits in your blood vessels, it may lead to following complications. 

a)Critical Limb Ischemia- Ischemia is a condition where there is less oxygen supply to the cells due to less blood flow. So, when there is any injury or infection in the limb & not treated for a long time it may leads to gangrene & amputation of limb is done. 

b)Stroke- Once atherosclerosis happened to the limb, it may not be limited to limbs only.Fatty deposits may also affect  your  heart  & brain. 

Diagnosis: PAD is easily diagnosed by following.

1.Physical Examination: There is weak or loss of pulse below narrowed area of artery. A whooshing sound will be there & cab be heard easily by stethoscope. Poor  healing of wound in affected area. 

2.Anti-Brachial Index: PAD is diagnosed by measuring the pressure in leg & arm and then comparing it..

3.Ultrasound: Doppler ultrasound technique is used to see the blood flow to the artery & also it is visualized the blockage arteries. 

4.Angiogram: Its a confirmatory test by which doctors easily diagnose PAD. An dye has been put into your blood stream and then X-ray has been taken out to see the blockage of arteries. 

5-Blood Test: Blood test should be done to find out the risk factors. 

Management & Treatments:

1. Diet:Try to take a healthy diet & maintain a healthy weight. 
2. Monitor Your Health Issues: Check out your existing health issues like  High Blood Pressure, Diabetes & Cholesterol & maintain at the optimum level because all of them are risk factors for PAD. 
3. Physical Exercise: Exercise regularly like 30 to 45 minutes 3 days in a week. Physical activity is the most important tool to make it better.Most of doctors recommend this to their patients.This simply increases the blood flow  to the extremities.
4. Stop Smoking: Smokers have 4 times higher chances to get PAD than non smokers. They have also high chances to get heart disease & stokes easily.

"Hemorrhoids & Homoeopathy"

 In the past few weeks there were many patients to whom I have consulted regaring their acute & chronic diseases.Most of them are better now and few are under the convalescence period. But among them there was a very interesting case which was really an inspiring case for us. Today we elaborately will discuss about that case. 

It was a case of Hemorrhoids. But before we discuss how inspiring that case was we should  have a solo knowledge about the diseased condition that patient was suffering from.

Hemorrhoids: Heamorrhoid is nothing but a varicose condition of the anerectal region  where veins become dilated,elongated &  tortuous. 

Types: Broadly it is categorised into 2 major types 

1)External:- In this type, veins are covered by skin. 

2)Internal:- In this type, veins are covered by mucous membrane. 

In some cases existence of 3rd type is also noticed called interno-external where upper part covered by mucous membrane & below one is covered by skin. And this is the advanced stage of heamorrhoid. Clinically it is seen that most of the heamorrhoids are internal one. 

Etiology: There is no exact reason for varicosity of veins but etiology for the same divided into 2 major groups A)Primary B)Secondary 

A)Primary

a)Anatomical Factors: Anatomically there are certain factors which accelerate the conditions.

1) Veins in anal region lax the sub-mucous coat  

2) There is no valve present in veins 

3) Veins flow against the gravity

4) Contraction of muscle during defeacation. 

b)Hereditary Factors: Congenital weakness may be sometime responsible for varcosity of veins. 

c) Exciting Factors: Exciting factors like long continued straining, contipation & colitis nay be act as exciting cause. 

B)Secondary 

1) Carcinoma of rectum

2)UterineTumor

3) Enlarged Prostate 

4) Portal Hypertension, systemic hypertension etc. 

Clinical Features:

Symptoms:-

-There must be a history of constipation.

-Patient complaints of blood coming with stool & sometime blood may be fresh so its called Bleeding Piles. 

-Condition is mostly painless but in some circumstances there may be pain. 

On Examination:-

1)Rectal Examination-In severe cases when veins getting heavy in varicosity, the adjacent mucosa hangs loosely from the anal opening & there may be anal prolapse also.

Depending upon the prolapse, piles graded into different degrees.

a)Ist Degree: No mucosal prolapse. 

b)2nd Degree: Prolapse due to defecation but returns back again. 

c)3rd Degree: Prolapse due to defecation but has to reduces manually .

d)4th Degree: Always prolapsed 

2)Digital Examination-Actually piles can be detected by the digital examination but it reveals the carcinoma of anus. 

3)Speculum Examination-With help of  proctoscope pies are easily visualized where exactly it is positioned. Basically piles are seen in 3,7 & 11 O'clock position.  

So, this is a short description about the piles to keep ourselves updated with this knowledge.Now lets come back to prime discussion how that patient did get  a magical cure.

At around 7 to 8 O'clock in the evening a handsome guy tall,fair in complexion with light hair color with red blushing skin just like imparting redness, entered in my clinic with his fellow friend. And without taking a formal permission he just took his seat along with his friend.After being comfortable in the seat patient's friend then started explaining the patient's complaints in the first instance. Then later on patient himself joined that conversation with me & tried to explain his complaints more elaborately. Till now his friend was giving just the overall complaints of the patient at very superficial level like burning at anus for last few weeks with pricking pain & little bit bleeding too, along with a small nodular mass at the end. Then patient gave an striking history of his complaints.

We the homoeopaths always search for the history because history is one of the most important point in the selection of a remedy, like history of injury , history of suppression etc.Anyway, then patient told me in very desperate way doctor, do you know? this is not the first time I have been suffering from this problem. I had been with this problem 6-7 yrs back also but that time its was very severe & that problem made me frightened & due to that I had to go for the operation at that time. But that was not my desire yet and I had applied multiple ointments also to avoid the operation, then I had found unfortunate one. 

Anyhow, doctor I d'not want to go for operation any more.Please do something for me doctor. This is the whole scenario that patient had expressed in front of me. Now the main task begins  like what would be the remedy & how we will going to select it?

After that I did take whole case history & then finally I prescribed Sulphur 30 two doses in sac.lac & placebo in globules for mental satisfaction of the patient. I also give the Calendular Q for sitz  bath for 2-3 times in day. After 2 weeks of consuming the prescribed medicines, patient did not give any response whether its helping him or not? Suddenly, after 3 weeks Patient came with whole family for the consultation. By seeing this scenario I assumed that patient would get some relieve only & that kind of expression patient had too. Anyhow I finished my consultation for his family. Then at last he came for his follow up, now he told me that doctor what a magical medicine you had given to me, its all gone now. NO burning, NO pain & NO any mass there. Its all gone forever.  You could not imagine how a doctor feel when his patient said that he is better, his problem recovered. But being a doctor we should always cross examine, only we fully believe whether this patient is saying right or not. After verification I found that  this patient is CURED now.
